Your First Visit Checklist
A little preparation goes a long way. Use this checklist to make sure you have everything you need for a smooth first appointment at Frontier Clinic.
01. What to Bring
Have these items with you when you arrive for your appointment.
Insurance card(s)
If you have coverage — bring all active cards.
Photo ID
Driver's license or state-issued ID.
All prescription medication bottles
Bring the actual bottles — not just a list.
Medical records (if available)
Recent labs, imaging results, or specialist notes.
Proof of income for sliding scale
Three recent pay stubs or a notarized employer letter stating household income.
Sliding-fee payment
Due at your appointment if you qualify for the discount.
Co-payment (if applicable)
For insured visits requiring a copay.
02. How to Prepare
A few steps to take before your visit to make the most of your time with your provider.
Complete your intake form
Fill out the online registration before your visit to save time.
Write down your medical history
Conditions, surgeries, hospitalizations, and family history.
List your current concerns
Note symptoms, when they started, and any questions for your provider.
Note allergies and reactions
Medications, foods, or environmental — and the reaction type.
Arrive 10–15 minutes early
Allows time to finalize paperwork and settle in.
Ask if fasting is needed
Some lab work requires fasting — confirm when scheduling.
Bring emergency contact info
Name and phone number of a trusted contact.
Good to Know
- Ages 16 and older — we focus on Internal Medicine.
- By appointment only, no walk-ins. Members enjoy same-day remote telehealth access, next-day in-person appointments guaranteed for founding members.
- Medicare patients are welcome — you'll sign a biennial waiver at enrollment.
- We do not prescribe controlled substances (opioids, benzodiazepines, stimulants).
- Lab work and imaging are billed separately at deeply discounted wholesale rates.
